This is a case report for a28-year-old healthy young man got infected by saprophytic gram-negative bacillus Burkholderia pseudomallei and caught by melioidosis. He was got infected during a military outfield training and showed cutaneous manifestations. The clinical presentation of melioidosis varied from patient to patient, and cutaneous presentations take about 13% of the cases. The usual histopathological features of cutaneous melioidosis are presented from suppurative to chronic granulomatous inflammation. This case show an unusual histopathological that there is no granulomatous inflammation happened.Instead lymphoplasmacytic inflammation was detected. This case warns the clinicians that cutaneous melioidosis should be suspect if patients with non-healing ulcers, even no typical clinical features shown out. The recommended treatment for the cutaneous melioidosis is a 2 week course of intravenous antibiotics (ceftazidimeor carbapenam) followed by a 3 month course of oral antibiotic such as cotrimoxazole.
